Private Lesson Request Form

Please print, complete, and return this form to your coach with $5 attached.  You will pay your private lesson fee directly to your coach the day of the lesson.

Birthday Party Request Form

Please print, complete and turn in your birthday party form to the front office for approval of your request. 

Participations Waiver

This waiver is required to be filed out and turned in to participate in any Umpqua Valley Gymnastic program, Birthday Party, Camp, Etc. If you are already registered for a normal class, it is not necessary to fill out again.

Vacation Request Form

Competition team families: Please fill out the vacation request form, and turn in to your coach a week before your vacation starts.

Scholarship Application

Please print, complete, and return the form to the front office.  A scholarship committee will review all applications, to review and determine any awards that are given based on need.
